This form is used to obtain Orphan Certificate. In case of death of both father and mother of a child he/she/guardian can apply for this certificate. It is considered as a proof for an orphan. This certificate is used to get entry to any orphanage for staying purpose and sometimes it is also required for higher education where there is any reservation quota for orphans. |
