This form is used both by District Industries Centre and the Entrepreneur for obtaining Memorandum for setting up of Micro, Medium or Small Scale Industries. Schedule 1 and Part I of this form is to be filled up by the District Industries Centre whereas Part II of the form is to be filled up by the entrepreneur. After filling up of the form it has to be submitted at the concerned District Industries Centre along with the necessary document required for this memorandum. Rules for filling up the form is also attached with this form. |
